<h3>What is a DMV examination?</h3>&nbsp;A DMV physical is a medical examination required for Commercial motor vehicle drivers by the department of Transportation (DOT). A DMV physical may be required annually or every 2 years while holding a transportation occupation. This examination evaluates if the driver is able to operate a commercial motor vehicle by reviewing their medical history, blood pressure, vision, hearing and collecting a urine sample.&nbsp;<h3>What documentation will you need to bring and how to prepare during your examination?</h3>&nbsp;Our office will provide you with the MCSA-5875 documentation to complete. If you have already filled out this form, please feel free to bring them in to your appointment.To help expedite the process during your visit, you may be required to bring additional records or item. We ask that you bring the following to your appointment if it applies:<ul style="list-style-type:disc;"><li>If you are taking medication, provide us with all your current medication or bring a list of medications with you.&nbsp;</li><li>If you are diabetic, provide us with your most recent laboratory test results to include A1C which recognizes how good your diabetes is controlled.&nbsp;</li><li>If you have sleep apnea and are using a CPAP machine, provide us with a printout of your sleep apnea machine usage within the last 30 days.</li><li>If you wear glasses or contacts, please wear or bring them with you as you will complete a vision test.</li><li>If you wear hearing aids and had a recent hearing test, please bring both items as you will complete a hearing test.</li><li>If you have a heart condition, provide us with a list of your medical history for our doctors to review. DMV Examinations

<h3>What is a Pilot Examination?</h3>&nbsp;An FAA physical is a medical evaluation of your medical history to determine if you are adequate to pilot an aircraft. The medical examiner reviews your physical, mental, and medical history. The provider has to be an approved aviation medical examiner. Dr. Patel has been serving as a Senior Aviation Medical Examiner for the Federal Aviation Authority.&nbsp;<h3>Preparing for your examination</h3>&nbsp;Our office offers 1st, 2nd, and 3rd class medical exams for pilots. During your appointment, the examiner will complete your medical examination. If all requirements are met, the provider will issue you a medical certificate. Depending on your class, An EKG may be required for ages 35 and older. An additional will may apply.&nbsp;If interested in completing a medical exam with our office, please follow these steps:&nbsp;<ul><li>Schedule an appointment with our office.&nbsp;</li><li>Before your appointment, you will need to register on <a href="https://medxpress.faa.gov " target="_blank" rel="noopener noreferrer">https://medxpress.faa.gov</a> &nbsp;and complete initiate your online application. We recommend that you print out a copy of the application to provide to the examiner during your medical exam.</li></ul>&nbsp;To help expedite the process during your visit, you may be required to bring additional records or documents. We ask that you bring the following to your appointment if it applies:<ul><li>For patients who wear glasses or contact lenses, we recommend that you schedule a visit with your optometrist before you visit our office and have them fill out the FAA 8500-7 form.</li><li>For patients with sleep apnea and are using a CPAP machine, provide us with a printout of the actual usage of the machine from the last 30 days.&nbsp;</li></ul>&nbsp;Please contact our office for any questions or to schedule your next appointment. &nbsp;&nbsp;

Pilot Examinations

Our office offers Immigration medical exams (I-693) for individuals applying for a U.S. green card or adjustment of status. The medical examination is required by U.S. Citizenship and Immigration Services (USCIS) to be performed by a civil surgeon. Dr. Jagdish Patel, M.D. has been a qualified civil surgeon for the last 30 years.&nbsp;<h3>What to expect during and after your initial visit?</h3> The doctor will perform a medical examination. This includes a review of your immunization and medical history. Our office will complete the required laboratory tests during your visit and results can take between 3-5 business days. If your tuberculosis lab test reveals a positive result, we will contact you to schedule a follow-up visit and complete a Chest X-ray. Once all the requirements have been met, we will have you sign and place the I-693 form with all records required in a sealed envelope. For a fixed price, applicants 14 years of age and younger, our immigration physical includes:&nbsp;<ul><li>Physical exam</li><li>Laboratory test: Tuberculosis</li><li>Vaccination and medical history review&nbsp;</li><li>A complete I-693 form with all required results is provided in a sealed envelope.</li><li>A copy of the I-693 for your records.</li></ul>For applicants 15 years and older, Our Immigration physical includes all of the above in addition to:&nbsp;<ul><li>Laboratory test: syphilis and gonorrhea</li></ul>The fixed price does not include required immunization or any treatment to treat or cure diseases that are required before submitting the forms. If you have any immunization records from home, we do kindly request that you provide our office with those records. If you don’t have proof, you can have a blood test to check for antibodies against specific vaccinations. Please visit the CDC website for further information on immunizations.<h3>&nbsp;</h3><h3>What documents will we need from the patient?</h3><ul><li>Valid government-issued photo ID: ID, passport, Driver's License, work permit.</li><li>A copy of vaccination records including the COVID vaccine (must be in English or English translation)</li><li>Health insurance card, if any&nbsp;</li><li>List of medications, if any</li><li>Previous medical records submitted to immigration (if applies)&nbsp;</li><li>Request for Evidence letter (if applies)</li><li>Immigration appointment notice letter (if applies)&nbsp;</li><li>I-693 pages 1-4 with form unsigned&nbsp;<a href="https://www.uscis.gov/i-693" target="_blank" rel="noopener noreferrer">https://www.uscis.gov/i-693</a></li></ul>In most cases, an appointment can be made within the same week. Immigration Physicals

<h3>What is Breath Alcohol Testing?&nbsp;</h3>&nbsp;Evidential Breath Alcohol Testing also known as EBT is a device that measures the alcohol level through the individual's breath. Our technicians are trained and qualified to perform the EBT and read accurately the result of this test. In some cases, and depending on the employer, this test may also be required with a drug test. These tests may also be used for reasonable suspicion, random selection, post-accident, pre- employment or work-related injury.&nbsp;<h3>What is a Urine Drug Test?</h3>&nbsp;A Urine drug test analyzes the urine sample for the presence of illegal drugs and prescription medication. In most cases, this test can be ordered by an employer. They will specify the test that we will perform during your visit in our clinic. We perform rapid urine drug testing in our lab and send out collections as needed.&nbsp;We offer a variety of drug tests including:<ul><li>In-house saliva and a rapid urine drug testing</li><li>Urine drug testing according to the DOT standards with the confirmation from NIDA approved laboratory.&nbsp;</li><li>Non-DOT urine drug testing can be customized according to the company needs.</li><li>Hair collections</li></ul>&nbsp;<h3>&nbsp;</h3>

Breath Alcohol and Drug Screen Testing

<h3>What is a Digital X-ray?</h3>&nbsp;A Digital X-ray is a non-invasive test that generates images from inside your body, primarily your bones. The images are then processed through our digital image capture system. The X-ray process may take up to 15 minutes. Results are provided to the provider immediately and discussed during of your visit avoiding multiple visits.During your appointment, our providers can promptly assess and provide accurate diagnosis after viewing the x-ray images and discuss appropriate treatment options.Please contact our office for any questions regarding our X-ray services.

Digital X-ray

<h3>What is Audiometry? &nbsp;</h3>Audiometry is a test that measures hearing acuity in a variation of sounds and pitch frequencies. This exam is performed to determine and identify how well an individual can hear. Tympanogram is an examination of the middle of the ear’s pressure. This examination traces the movement of the membrane. Combinations of these exams are helpful in determining if there is any hearing loss.&nbsp;&nbsp;<h3>What occurs during and after the hearing exam?</h3>&nbsp;The exam is conducted in an isolated room restricting incoming noise. We will provide you with headphones that are connected to a device that produces tones and different sound to your ears including a button that we will ask to press when you have recognized a sound. During the tympanometry exam, the technician will inspect the ear canal to verify there is no obstruction.&nbsp;After the exam, the doctor will provide you with your results. The results may also be sent to your employer if your visit was pertaining to your employment.

<h3>What are Immunizations?</h3>&nbsp;Immunizations are a process that produces immunity or resistance through vaccination to protect from specific disease or infections. For our occupational medicine services, we offer a variety of immunizations. Laboratory Services

<h3>What is First-Aid Treatment?</h3>&nbsp;First aid treatment is medical attention given to an employee who has been injured, became ill, or experiences other medical concerns during their work hours. Usually, consists of a one-time visit and does not require the employee to have time off from work. First aid treatment is given to the patient prior a more thorough medical attention may be needed. First aid includes but not limited to cleaning, bandaging, dispensing non-prescription medicine, administering immunizations, and removal of debris regarding to the injury.Emergency rooms cost can be outright expensive. Our office works with employers providing quality care to their employees and cost-effective treatment.Per OSHA, first aid treatments are not required to be recorded. In the event that the patient needs further medical care, the employer would be required to report and record the work-related injury. First Aid Treatment

<h3>What is a Fitness for Duty evaluation?</h3>&nbsp;A Fitness for Duty evaluation (FFD) is used to determine if the candidate is physical and mentally suitable for the job. Employers rely on these exams to ensure their employees can safely perform their specific job requirements. Some of the reasons for this evaluation are but not limited to an employee returning to work after a personal illness or work injury, baby bonding, and safety concerns while employed. Fitness for Duty Evaluation

<h3>What is a Job Placement Assessment?</h3>&nbsp;A job placement assessment is an examination by a provider who evaluates whether the candidate can meet the job requirements asked by the employer. Typically, a job placement assessment is for new hires or candidates offered employment opportunities and the examination will determine the final decision in their hiring process. Employers want to ensure the candidate has the capacity and skill set to perform their potential job duties. In other circumstance, a patient may be changing job titles within the same company and will require a job placement assessment. Our office offers custom examinations to meet your company needs.In our area, most of the occupations are offered in a warehouse setting. Our most common evaluations we offered are as following:1. Cardiovascular fitness: We utilize a step test which includes climbing up and down over 12" inches high stool at 42 times per minute. This gives us the heart rate baseline. After the exercise, we also examine the conditioning of the person at the time of evaluation.&nbsp;2. Upper body strength and fitness: We utilize 5 pounds dumbbell, various movements of the wrist and elbows, and push-ups.&nbsp;3. Incremental weightlifting: We start with 5 pounds from floor to the waist up to 100 pounds and from the waist to above shoulder of 50 pounds.&nbsp;Based on the performance, we can assess how the candidate will lift on occasional, frequent or rarely.If you are interested in registering your company, please contact our office or send us an inquiry through our contact form. &nbsp;

Job Placement Assessment

<h3>What are Work-related injuries?</h3>&nbsp;Work related injuries are when an employee injures, or an illness is developed whether physical or psychological during the duration of their employment. The employee is responsible to report the injury to their employer immediately. If the injury is deemed to be work-related, the patient is entitled to workers’ compensation benefits. Workers' compensation is a form of insurance that provides a claim adjuster to handle the claim, wage replacement and travel reimbursement. Depending on the insurance carrier, they may offer other benefits such as translation, transportation and more. There are no payments (i.e., co-pay, co-ins or deductibles) collected by the patient for their medical care.At Premier Urgent Care, we thrive on providing the best care and getting the patient back to work. Below you will find our condense explanation:<ol><li>During the patient’s visit, our providers provide an examination and treatment. This develops a Doctor’s First Report (DFR) that must be provided to the workers compensation carrier within 5 days of the patient’s visit. The report provides patient demographics, description of injury causation, doctor’s examination notation, treatment provided and if a referral was placed.&nbsp;</li><li>Our practice obtains workers compensation insurance information for the employer. It is the employer’s responsibility to report and file the claim and provide us with claim information to proceed with further care. This information is crucial from the very beginning of the treatment. The required information consists of: Claim number, Claim adjuster’s contact information, Workers' compensation insurance name, billing address and contact information. We also recommend that the patient provides us with that information in the event that they received it prior us being notified.</li><li>If additional services are required beyond what our office provides, we will generate a Request for Authorization also known as an RFA. Authorization from the workers compensation insurance is required for the following services but not limited to therapy, specialist visits, medication, advance radiology and injections. The complete process from generating the request, faxing the documentation to the workers compensation insurance, receiving the determination and generating the referral if approved may take between 5-14 business days.</li><li>Once faxed, a copy will be provided to the patient. Please note some specialists do review records prior to scheduling an appointment with the patient. Although a referral has been sent it is not a guarantee that the specialist will approve your referral. If this occurs, please contact our office to redirect the referral to another provider in your workers compensation medical provider network.</li></ol>Our providers are contracted with a variety of workers compensation insurances to provide quality care and ultimately assisting the patient to return back to work in a safe and timely manner.
