DMV Examinations

What is a DMV examination?

 

A DMV physical is a medical examination required for Commercial motor vehicle
drivers by the department of Transportation (DOT). A DMV physical may be
required annually or every 2 years while holding a transportation occupation. This
examination evaluates if the driver is able to operate a commercial motor vehicle by
reviewing their medical history, blood pressure, vision, hearing and collecting a urine
sample.

 

What documentation will you need to bring and how to prepare during your examination?

 

Our office will provide you with the MCSA-5875 documentation to complete. If you have already filled out this form, please feel free to bring them in to your appointment.

To help expedite the process during your visit, you may be required to bring additional records or item. We ask that you bring the following to your appointment if it applies:

  • If you are taking medication, provide us with all your current medication or bring a list of medications with you. 
  • If you are diabetic, provide us with your most recent laboratory test results to include A1C which recognizes how good your diabetes is controlled. 
  • If you have sleep apnea and are using a CPAP machine, provide us with a printout of your sleep apnea machine usage within the last 30 days.
  • If you wear glasses or contacts, please wear or bring them with you as you will complete a vision test.
  • If you wear hearing aids and had a recent hearing test, please bring both items as you will complete a hearing test.
  • If you have a heart condition, provide us with a list of your medical history for our doctors to review. Any recent study will be helpful for the medical examiner.
